Role of Adult Services in Changing Environment
This workshop presentation provided an overview of a model that was developed through the expertise of The Child and Family Policy Institute of California, California Social Work Education Center and Alameda County Social Services Agency. The presenters shared promising practices in coordinating care for aging and older adults and described the optimal use of social workers in a team-based approach to provide the highest quality of care for older adults along a continuum a physical and psychosocial needs. Alameda and Riverside counties have both implemented their tailored version of the model and will share their successes, challenges, lessons learned and next steps to continuous practice improvement.
